Alternatively you could print the Health questionnaire out and bring it with you on the day of your appointment.
Cancellation notice!!! If you cannot make your booking please call, text or e-mail us 24 hours in advance, this is important as the therapist makes that special time for you; empty appointments could go to someone else.
